    General Health Appraisal & Immunizations
  • Children's Garden and The State of Colorado require several forms to be filled annually for each child attending the school.  In this document, you'll upload the following forms:

    General Health Appraisal (GHA)
    Immunization Records
    Emergency Medical Care, Asthma Care Plan (if applicable)

    All forms must be current.  GHA forms must be current within the last 12 months.  When you visit for your child's annual check-up, you can request an updated copy for our records.

    Your doctor may email you these forms directly, hand you a copy, or upload them to a digital chart.  There are instructions to find them on MyChart on the following page, which may be helpful.

  • GENERAL HEALTH APPRAISAL

    Find your child's profile and in their side menu, tap/click "Letters".  From there, tap the GHA and "Print".  Instead of the Destination being a printer, tap to save as a PDF:

    Note: Be sure this is current within the last 12 months, and make a note if you'll need a new one before August or Summer Camp, if you're enrolled!

    IMMUNIZATIONS

     

    1. Find your child's profile and in their side menu, tap/click Health Summary, then tap/click Share My Record.

     

    2. Next, tap/click "Anyone Else" followed by "Request a Formal Record".

    If you use Children's, you can click this link to get there (opens in new tab).

     

     

    3. You'll request your doctor's office, or Children's, to send you a formal record of immunizations. Be sure to select "All Available Dates" and to include Immunizations.

    This will come back to you in PDF form via MyChart or email, and you can upload it on the next page.

  • Alternatively, you may upload an Exemption Form for vaccines, if you have one. For more information, please see: https://cdphe.colorado.gov/exemptions-to-school-required-vaccines

  • Please note: These forms are required by the child's first day of school (or summer camp, if applicable), per state law.  Thank you for your help in getting these in in a timely manner! 

    You can come back to this form to upload more documents if needed by clicking the link again in your email.

  • Emergency Medication

  • Please indicate below whether your child has any health circumstances requiring emergency medication to be kept at school.

    Examples: Asthma, anaphylactic food allergies, diabetes, FPIES, etc.

     

    Note: If your child requires medication to be kept at school, you will need to upload an additional form (Emergency Medical Care Plan for anaphylaxis, Asthma Action Plan, or other relevant form).

    Medication can be brought to the school ahead of the first day, during staff set up week.  Please note that this medication must be at school (or summer camp) for your child's first day, per state requirements.  Thank you!

  • Will your child require Emergency Medication kept at school for a health condition (asthma, anaphylactic allergy, diabetes, etc.)? Note: this will require an additional form, signed by your doctor, which you can upload below if you have it.*
